Zandronum now allows to have "teams" in Cooperative game modes. It is confirmed to have been showing "ERROR" message in Doomseeker, and they seem to have fixed it by now. Apparently the same shows in Doom Explorer as <BAD DATA>

Quote by an author of the mod:
"Small update on Zombie Horde 2 servers showing up as "<ERROR>".
We suspect Zandronum either sends malformed data, or Doomseeker has an error attempting to parse the incoming data. But what would cause that to happen?
Well, Zombie Horde 2 has a special feature where it uses the cooperative gamemode, but it enables teams. This behaviour is defined here in a GAMEMODE lump. It is likely that Doomseeker doesn't expect cooperative gamemodes to send teams, and causes it to show an error. Thanks to Kaminsky who figured out the possible reason.
I have reported the issue to Zalewa, who develops Doomseeker. If the issue is fixed on his end then there will be a need to update Doomseeker to play the mod properly. Otherwise the server might not show up correctly.

UPD: This has been fixed! Doomseeker has just released v1.5.1-p1 which is an exception to the regular release schedule to fix this specific bug to Zombie Horde 2 servers."

Seems like it happen when server is active (players in game), perhaphs it triggers a team mode once people join.

UPD Kaminsky's quote:
How Doom Explorer treats game modes (e.g. cooperative isn't a team-based game mode) is likely the same as how Doomseeker does, as explained here: https://zandronum.com/tracker/view.php?id=4571#c24623
